Understanding the implementation of COVID-19 surveillance and control measures across schools in England

We are researchers from Public Health England. We are contacting you because the school that your child attends is participating in the sKIDs (also called COVID-19 Surveillance in KIDs) and sKIDsPLUS surveillance studies to understand COVID-19 transmission in primary and secondary schools. As a part of this work, we are examining parents’/guardians’ views on the implementation of measures to prevent transmission of Covid-19 in schools and their experiences with the sKID study.
To do so, we are requesting that all parents/guardians in your child's year group at school fill in a questionnaire. Please complete only one questionnaire per household.

All the information you provide will be treated as confidential. We will not ask for your name or the name of your child(ren). Any reporting of the data will be in anonymous form. So please answer all the questions as fully and honestly as possible.

It is up to you whether you want to complete this questionnaire and you may stop taking part at any time or skip any questions you do not want to answer with no negative consequences.
This questionnaire will take approximately 5 minutes to complete.
